Group Leaders’ Information:
10 things to know about….$500
for 4 people - and $250 for 4 other people

You must sign-up by January 30, 2008 - and let us know that you want to be part of the contest.
- You will need to have at least 6 people in each group with a maximum number of 12. We will be rewarding the top 4 people who teach the most "new" people in 2008. The groups that are eligible are RFL 1, RFL 2, WRAP, and Journaling.
- You must fill out a meeting record each time the group meets. Then you need to fax or mail the record after the 5th lesson.
- Each group member needs to complete a participant Information sheet.
- Each group member needs to complete a consent for Participation/Confidentiality form.
- Each group member needs to complete a consent for participation in Evaluation form.
- Each group member needs to complete a pre-test.
- You must fax or mail
in the completed meeting record after you complete the group.
- Each group member needs to complete a post test.
- You must provide us an action plan for what you plan to do with the money if you win.
